Veterinary Clinic of Symbioun Inc. provides our forms online so that you can fill them out in the comfort of your own home and at your convenience!

Notice:

A $50 handling fee will now apply to all semen retrieval shipments and incoming semen shipments.

Semen storage fees will increase from $10 to $15 per vial/straw.

Our office will now be closed from 12pm-1pm on weekdays to ensure our staff has a lunch break.
